This section only applies if you are a U.S. taxpayer and if AIARC processes payroll for your Center.

If you are a U.S. taxpayer, AIARC will prepare Form W-2 (Wage and Tax Statement) for the calendar year in which you last worked. AIARC will provide you a copy of your Form W-2 to the email address that is on file.

If you request AIARC to provide a copy of your Form W-2 to an email address that differs from the address on file, complete a new Contact Information Separation Form and send a signed copy of the completed form to your center's coordinator.
